Correct option - 3

Here, concept of passive voice is brought under consideration. Since, we know helping verb has is always followed by verb's 3rd form. Thus, option 1 is clearly eliminated. Yesterday is used here, shows past tense, thus, option 2 is eliminated. Hence, option 3 i.e. was supposed is the correct answer.

The correct form of passive voice in simple past tense is :-

Object + was/were + v3 + subject.

Correct sentence  - The train was supposed to arrive at 6 pm yesterday.
