There is no error in the sentence. Hence, the answer is option 4.

Part C is correct because $‘in’ $is used as a substitute for the word ‘during’. $However, another variant of the same sentence (which is also correct) would be to say, ‘The teacher asked her what grade she got on $the test’. Both variants are used frequently and one can even use the word ‘during’ instead of ‘in’ when referring to a test.

Thus, this sentence needs no change.
