Correct option - 2

Here, concept of adjective is brought under consideration. Since, truly is an adverb, an adverb is a word or phrase that modifies the meaning of an adjective, verb, or other adverb, expressing manner, place, time, or degree (e.g. gently, here, now, very). But, exception is a noun and an adverb is not used before noun. Also, talent is a noun. We know adjective is used before noun. Hence, in order to make 'exception' an adjective, replace exception with exceptional. Thus, option 2 has grammatical error. So, option 2 is the correct answer.

Correct sentence - Pablo Picasso showed his truly exceptional talent from a very young age.
