Correct option - 2

Since, modal verb may is used in part 2 and then followed by verb hurt and connected with a conjunction or, so the modal verb applies on second verb mistreat also. Hence, replace mistreats with mistreat. Thus, option 2 is the correct answer.

Correct sentence - It is easy to fall victim not only to those who may hurt or mistreat us, but also to our own anger.
