The error lies in part A and D.

In A the article 'the' is missing before the word 'damage'. It is something specific thus the article must be used here. 'It is worth to mention that the damage..'

In D the verb 'existing' is incorrect and must be replaced with the noun 'existence' to make the sentence correct. 'rupture, is involved in all aspects of the huma$n existence.'$

Correct sentence:$

It is worth to mention that the damage caused by the climate change, from $natural disasters to food chain rupture, is involved in all aspects of the human existence.$
